Student Enrollment Records

All parents are expected to follow the required enrollment guidelines, and provide the school with the documentation that is required by the state and school for each child. It is extremely important that all information be valid and current. If you move or change telephone numbers, you must notify the administrative staff. Always include a working telephone number and emergency contact number. The school must be able to reach you should there be a medical or other emergency which involves your child. In addition, a “Parent Authorization for Release Form” must be completed and on file for each student. This authorizes the school to release students only to those listed on the form. If a name is not listed on this form, the student will not be released to that person. Please forms below in order to register your child.
